Two answers. For your lane: the champion that sits in the most “worst matchup” lists of that lane, the counter you meet most often (in the top lane on patch 26.21: Garen, in 9 champions' lists). For your pool: the champion that is a hard matchup for most of what you play (for a Sett player: Nasus, the most picked of Sett's worst matchups). The games behind this site hold no ban rates, so this page counts counters, and says so.
Best bans per lane (patch 26.21): the counters you meet most
These are not ban rates. The games behind this site record picks and results, not bans, so this table counts something else that is useful for the same decision: how many champions in the lane have this champion among their worst matchups. The champion at the top of a lane's table is a hard matchup for the most picks there, which makes it the ban that protects the most of your pool when your pool has no bad matchup in common. Every champion links to its counter page.

The whole lane, with the counters of every champion, is on the counter pages; the ranking is on the tier list. Per rank: the tier lists get a rank filter once a rank group has enough games; the counters are counted over all ranks together, because one rank has too few games per matchup.
Who to ban as your main
The question people ask on every counter page: “Who should I ban as Garen?” For the ten most played champions of each lane on patch 26.21, the answer by the data: of the opponents the champion does worst against (at least 8 games in the matchup, a win rate below 50% after it is pulled toward 50%), the one picked most in that lane. That is the counter you meet most often, and the ban that saves you the most lanes. The pair link opens both sides of the matchup.

Ban for your pool, not for the meta
A ban is one champion out of the game for both teams. The meta ban removes a champion that is a little strong against everyone; the pool ban removes the champion that is very bad for you. Look at the three champions you play most and their worst matchups on their counter pages. If two of the three lose to the same champion, that is your ban, and a reason to add a fourth champion that handles it. If your pool has no bad matchup in common, your pool is already doing the ban's job, and you can spend it on the lane's most common counter from the first table, or on what your team asks for. How to build a pool that covers itself: your champion pool.
Ban or counter pick?
Your spot in the pick order decides it. As last pick you can answer a champion instead of banning it: if the enemy top laner shows, you take the counter from your pool, so the ban goes to the pick your team cannot handle. As first pick you cannot answer anyone, so the ban protects your blind pick: remove the counter you meet most. The two guides: counter pick instead of ban and what to ban with your pick order.
Bans that are not about strength
Some champions get banned far more often than their win rate explains: they are annoying to play against rather than strong. Without ban data this page cannot measure which, and it does not quote other sites' ban rates. The rule still holds: a frustration ban is a fine ban when nothing in your pool loses to a common champion, and a wasted one when something does.
In champ select
LazyElo does not suggest bans. What it does once bans are in: banned and picked champions drop out of your pool, and it scores what is left against the picks you can see, your own record and what your team still needs, then puts the best pick first with the reasons. The free plan shows your best 3 picks. The feature: your pool after bans, in champ select.
The numbers: 5,462 ranked games on patch 26.21. Games shared anonymously by LazyElo apps, recounted every hour. All ranks together; your own rank can differ.
Questions and answers
Who should I ban in LoL?
The champion that is a hard matchup for most of the champions you play. If two of your three champions lose to the same opponent, that is your ban. If your pool has no bad matchup in common, ban the champion that counters the most picks in your lane; the first table on this page names it per lane.
Who should I ban as Garen?
Yone. Of the champions Garen does worst against in the top lane on patch 26.21, it is the one picked most (4.8% of top picks, 520 games), so it is the counter you meet most often. Each of Garen's worst matchups has at least 8 games behind it.
Who do I ban as Thresh?
Nami. Of the champions Thresh does worst against as a support on patch 26.21, it is the one picked most (3.4% of support picks, 375 games), so it is the counter you meet most often. Each of Thresh's worst matchups has at least 8 games behind it.
Is it better to ban a counter or a meta pick?
A counter to your own pool, nearly always. A meta pick is strong against everyone a little; your counter is bad for you a lot, and nobody else on your team will ban it for you. The exception is when you are last pick and can answer the counter with another champion you play: then the ban is free for the pick your team cannot handle.
What is the most banned champion right now?
This site cannot say: the shared games behind it record picks and results, not bans, and we do not copy ban rates from other sites. What it can say is which champion counters the most picks in each lane this patch, which is the ban that helps your lane most; see the table per lane above.
